About The Role
Liaising with the Care Manager and Residential Services Manager, you will provide leadership, guidance, coaching and mentoring to Nurses and Personal Carers to ensure a holistic person centred service and support process, that aligns with the SCC Better for Life model, is adopted and is safe and effective and tailored to meet each resident’s needs, goals and preferences.
Key Responsibilities Includes But Are Not Limited To
  • Working positively and collaboratively with other staff and qualified practitioners to provide proactive, holistic and person centred service and support, that is safe and effective, for residents
  • Listening to residents (and their representatives) about their needs, goals and preferences and work collaboratively, and in partnership, with them to develop a safe and effective care and service plan
  • Maintaining current knowledge of contemporary best practice clinical care and maintaining records of professional development to meet AHPRA requirements
  • Coordinating the on boarding process for all new care and nursing employees plus agency staff to ensure an appropriate orientation into SCC’s Better for Life model, policies and procedures appropriate for their role and an understanding of their job requirements
  • Monitoring SCC’s processes to ensure nursing and care staff read and comply with all policies, procedures and work instructions relevant to them
  • Meeting and exceeding all relevant standards, regulations and laws, and contributing to our continuous improvement plans

The successful candidate will have the following skills and experience:
  • Current registration (practising certificate) as a Registered Nurse with AHPRA
  • Current First Aid Certificate (HLTAID011) & Current CPR Certificate (HLTAID009)
  • Previous experience working as a Registered Nurse with supervisory, leadership responsibilities in an aged care environment
  • Demonstrated best practice clinical management skills appropriate to the role
  • Sound time management skills in order to manage own workload to meet deadlines and to support other staff
  • Outstanding decision making, negotiation and problem solving skills to balance supporting resident’s desire for independence and self determination to make their own choices against potential risks and duty of care
  • Comprehensive understanding of the requirements to support residents living with dementia including those who display challenging behaviours
  • Excellent knowledge of the Aged Care Standards particularly as they relate to accreditation

About the role

Key Responsibilities

  • Administration of IV, IM, SC and PO medications
  • IV Cannulation, PICC line and Infusaport care
  • Simple and Complex wound care including compression and NPWT
  • Drain care
  • Male, Female and Supra Pubic Catheter Care and Education
  • Accurate/Timely documentation in all client records.
  • Appropriate communication within the clinical team to ensure appropriate care is provided.
  • Complete initial assessments/develop care plans to ensure best outcome for our clients.
  • Monitor and update care plans to ensure appropriate care is provided
  • Deliver education to client and families to enable best possible outcome.
  • Recognise and respond to clinical deterioration, with appropriate escalation.

  • Minimum 1 year acute experience
  • NDIS Clearance (or willingness to obtain one)
  • Police Clearance within 6 months (or willingness to obtain one)
  • Use of own car – KM allowance provided

  • Support and education for clients with terminal diagnoses

  • Administration and preparation of s/c medications and CSCI for symptom management
  • General nursing care including simple and complex wound care, IDC/SPC care, PICC dressings

  • Appropriate communication within the clinical team and community palliative care teams and referrers to ensure care is provided.
  • Complete initial assessments/develop care plans to ensure best outcome for our clients.

  • Promoting best practice and initiating quality improvement activities
What You Bring
  • Current AHPRA Nursing
  • Minimum 2 years palliative care nursing experience, community experience desirable
  • Post graduate qualifications (or working towards) in Palliative Care
  • NDIS Clearance (or willingness to obtain one)

The Ideal Candidate: The ideal candidate for our team will have at least 2 years experience with managing client enquiries, client consultations, client onboarding, and a client caseload for Home Care Package , NDIS, and private pay clients. The care coordinator will have experience managing the complex scheduling requirements for community care workers.

Purpose of Position: This position places a strong emphasis on prioritizing customer satisfaction and involves engaging with families, conducting in-home care assessments, and formulating customized care plans tailored to meet the specific needs of our clients. The role also encompasses supporting administrative personnel in various aspects such as recruitment, training, and overseeing the performance of our caregivers and nurses.

Key Responsibilities:

Provide compassionate and client-centred support by responding to a high volume of phone and online enquiries, ensuring timely and empathetic communication.

Meet with clients to understand their unique needs, complete in-home care assessments, and develop tailored care plans aligned with Aged Care Quality Standards.

Address a wide range of day-to-day client matters, including general enquiries, feedback, service adjustments, and complaints, with professionalism and a solution-focused approach.

Support the Rostering team by assisting in the creation of master rosters and performing scheduling tasks as required to ensure efficient service delivery.

Manage daily scheduling changes and ensure care is delivered according to client needs and staff availability.

Support the care team through training, mentoring, and performance monitoring to ensure high-quality, person-centred care.

Conduct staff assessments and assist in credentialing care workers for medication administration, fostering a skilled and confident team.

Maintain accurate and compliant documentation for both clients and staff, in line with government regulations and organisational policies.

Demonstrate adaptability and a proactive mindset by embracing change and taking challenges as opportunities for growth.

Participate in a rotating “on-call” system to ensure clients and staff receive timely support after hours, on weekends, and public holidays.

Qualifications

QUALIFICATIONS:

Possession of current AHPRA registration (RN) (specialized in Ageing or Disability & Community )

Current Australian (Satisfactory) National Police check, Working with Children Check,

Current First Aid & CPR certificate.

Driver's license and your own reliable registered vehicle

Excellent verbal and written communication skills

Professional Indemnity Insurance (RN)

A smart mobile phone (iPhone / android)

Preferred Skills

Preferred Skills :

At least 2 year experience as a scheduler, case manager, care coordinator ( Home Care Package ) and hospital discharge planner

About The Role
We are currently seeking a dedicated Registered Nurse to join our team at Fullarton providing exceptional care and support to the residents. This will be a stable, Monday-to-Friday role (0700 to 1515) with no weekend or night shifts.
This position is ideal for someone who enjoys a structured routine, values meaningful connection with consumers, and thrives in a team that supports each other.

What You’ll Do
  • Daily clinical management of 15 CAP consumers
  • Supporting a nearby area with EN guidance and back-up
  • Assisting the Care Coordinator with documentation, planning, and leadership
  • Leading or supporting new consumer admissions
  • Communicating clearly with families, allied health and other care partners
  • Foster a welcoming, inclusive environment for residents
  • Actively engaging in quality and risk management is essential for meeting standards and promoting ongoing improvement.
What You’ll Bring:
  • AHPRA Registration as RN with experience in aged care or subacute care
  • Familiarity with Lee care is preferred; however, a demonstrated ability to learn new systems quickly is also acceptable
  • Able to keep things running smoothly, even during busy times.
  • Values good team culture and shared responsibility
  • Organised, motivated, and reliable, with the ability to work both independently and as part of a team
Special Requirements
  • This is a NDIS risk-assessed role – a current NDIS Worker Screening Clearance is required
  • Recommended to have current vaccinations in line with government health directives (e.g. COVID-19, influenza)
  • Must complete a pre-employment medical assessment as part of the recruitment process
